Font of Choice: I'm using Impact and Hand of Sean.

WRITTEN IN PSPX1

Open a new 600x600px canvas and flood fill white.
Open the background image and paste on new layer.
Now add your frame.
Open a pattern paper, resize 50% and paste.
Place your paper behind the frame and erase excess.
Now open your first tube and place behind frame.
Erase excess.
Add your elements where you wish.

Add your 'Sexy Girl' text on heart element.
Now you add your main tube above all other layers.
Directly under your tube add your name.
To give the distorted look in my tag get your pick tool
Have your mode set to perspective and drag outwards.
Add your dropshadow and DON'T FORGET YOUR COPYRIGHT!!

Crop off excess and save with or without white background as PNG
